Report into death of toddler is delayed

A public body which protects children has said lessons have been learnt from the death of Belper toddler Riley Pettipierre.

The Derbyshire Safeguarding Children Board made the claim as it was revealed that a report into Riley’s death had been delayed.

The board is currently putting together a serious case review into the death of Riley, who was killed when he drank methadone from a child’s beaker.

The report, which details the circumstances leading up to Riley’s death, to see if it could have been predicted or prevented, was supposed to be completed at the end of January. It is now expected in the”next couple of months”, as the agency said it would not publish until “all investigations are complete”.

However, a spokesman said the board - which includes senior members of the NHS, local authoirites and police - had already made changes to prevent similar deaths from occuring again.

A spokeswoman said: “We’re keen to ensure that the final report and its recommendations take account of all relevant information leading up to Riley’s death.

“We also need to look in detail at the information presented to the court which may not have been available prior to the court hearing. There is, therefore, some further work to do on the final report.

“However, we have not waited for the outcome of the report to change and improve the way we work together to support families like Riley’s.

“Our services are now better coordinated.”

“We will publish the recommendations as soon as we receive the completed seriouscase review report - expected in the next couple of months.”

Riley’s parents, Sally Dentand Shaun Binfield were found guilty of his manslaughter last month following a ten day trial.

Dent had been on a methadone prescription before becoming pregnant and also used heroin and crack cocaine.

Dent had poured methadone into a child’s beaker, which she and her partner, Shaun Binfield, left in the reach of the two-year-old.

The pair will be sentenced at Nottingham Crown Court on February 19.
